Summary:
This position is a Grant Funded position with a minimum of a 4-year employment term (02/2023 - 9/30/2027).
Position may be extended as more funding is secured.
Under the direct supervision of the Grant Writer in the Behavioral Health department, the Prevention Program Coordinator & Outreach Specialist will manage, coordinate and implement various grant funded initiatives based on funding awarded to Navajo Health Foundation/Sage Memorial Hospital.
Program areas may include programs that address opioid abuse, chronic disease prevention, HIV/AIDS, tobacco use prevention, mental health awareness, physical activity promotion, senior health and wellness, injury prevention, suicide, and other public health prevention programs.

Essential Duties &
Responsibilities:
Serve as the Project Director for various grant-related initiatives and serve as the point person for funders.
Plan, coordinate, and implement grant-related program activities and develop strategic plans to outline strategies and tasks.
Work closely with different SMH departments to coordinate grant funded programs.
Research, develop, and/or recommend revisions to policies, procedures and/or protocols as they relate to selected service delivery models or evidence-based practices/strategies chosen for the grant programs.
Facilitate the approval process among committees/task force/teams, departments and hospital leadership of policies, procedures, and protocols.
Conduct organizational and/or community assessments to identify gaps in services.
Coordinate and implement programs to increase family engagement, protective factors and recovery support based on needs assessments.
Engage individuals with lived experience in program activities.
Perform a training needs assessment and design a training plan, complete with schedule, for departments and/or community partners.
Obtain training and master certifications in various evidence-based practices or service delivery models selected for grant programs.
Develop training curriculums and conduct training in the community or among SMH employees on evidence-based practices, service delivery models or approved/updated policy, procedures and protocols to ensure consensus among departments and community partners.
Build and foster community partnerships (organizations, businesses, schools, etc.
) and engage them in policy/procedure/protocol discussions, program development and other programmatic activities to meet grant objectives.
Form, lead and coordinate committees, task forces, and/or teams to accomplish grant objectives.
Establish Memorandum of Agreements, sub-award agreements and/or contracts with community partners as needed.
Prepare and give presentations (in-person and virtual) to SMH staff, the general public, schools, local government, businesses and professional groups on grant funded initiatives and/or programs.
Develop a Prevention Strategy Plan in collaboration with SMH departments and partners and implement evidence-based social marketing, awareness campaigns, and public messaging (PSAs), as needed, to increase community awareness and access to programs for youth, adults and families.
Develop or select appropriate health education materials or resources to be used for programs.
Design flyers, brochures, and/or posters to increase community awareness of public health issues and promote community events and training opportunities.
Collaborate with partners on local, tribal, state, and national prevention efforts.
Oversee the implementation and installation of tools and/or software needed for grant-related projects, i.
e.
, Meditech EHR toolkits.
Develop a data collection plan for grant programs and collect, record, maintain, and analyze program data.
Create and collect surveys and utilize results to improve health education activities/programs and report to stakeholders.
Evaluate program effectiveness, reporting results to stakeholders, and make recommendations to supervisor and/or hospital leadership to improve programs.
Plan, organize, implement, and/or participate in grant-related special events, community meetings, focus groups, workshops, in-service trainings, conferences, and funder meetings, as they relate to objectives and goals of various awarded grant funding.
Recruit, coordinate and supervise volunteers for community program implementation as needed.
Conduct research and recommend strategies to improve grant-funded programs.
Supervise hired grant program staff.
Write formal letters, meeting agendas, grant summary/data reports, etc.
Work with various departments to utilize existing resources to carry out grant activities Ex:
Working with IT to announce community grant activities on website.
Adhere to SMH policies and procedures.
Coordinate services (quotes, payment, schedule) with outside vendors, contractors, etc.
, to fulfill program activities and practice responsible grant expending.
Monitor grant-related expenditures and remain within approved budget limit.
Participate in discussions and assist with program planning for future potential grant funding programs or succeeding grant terms.
Adhere to the Uniform Administrative Requirements, Cost Principles, and Audit Requirements for Federal Awards for all federal grant programs and other funder requirements.
Prepare and submit required post-award, programmatic and financial reports to funders in a timely manner.
Adhere to SMH policies and procedures.
Other duties to accomplish the goals and objectives of the grant related project.
Knowledge, Abilities, Skills, and Certifications:
Knowledge of health issues impacting Navajo Nation communities and interventions used to prevent those health issues.
Knowledge of principles and practices of health education, social and behavioral sciences, and health behavior change as they apply to public health; mental health, or human services; or physical and biological sciences as they apply to the environment.
Knowledge of methods and uses of mass communication, media, social marketing, and media advocacy.
Knowledge of Microsoft suite software and various social media platforms and the demographics that use them.
Knowledge of statistical concepts and methods for collecting, compiling and interpreting data.
Knowledge of principles, methods, techniques, theories and practices of health education and public health or human services.
Knowledge of underlying determinants of health and risk factors for disease/injury, or social welfare principles and practices.
Knowledge of effective outreach and community engagement strategies.
Knowledge of principles and practices of program management, including funding, developing and evaluating program quality and effectiveness.
Ability to demonstrate effective written and verbal communication skills.
Ability to establish and maintain effective working relationships.
Ability to provide leadership to community coalitions or collaboratives.
Ability to operate a personal computer and use common software.
Ability to write or edit clear and effective reports, correspondence, and educational materials.
Ability to work independently, meet deadlines and work under pressure.
Ability to interpret health related data and communicate it to the public in a meaningful way.
Ability to interpret federal, state, local laws and regulations and applicable data relating to specific health issues (e.
g.
, tobacco, injury prevention) or social service programs (e.

Ability to develop and conduct effective presentations or trainings for school, professional and community groups.
Ability to design or review brochures, advertisements and newsletters.
Ability to evaluate effectiveness of programs.
Experience in successfully educating youth and adults through presentations, written materials, and media and social media messaging.
Experience in grant program planning, implementation and assessment.
Experience in policy, procedure, and protocol development and revision.
Experience in developing planning, implementing, evaluating or overseeing community education or outreach programs or activities.
Experience in supervising staff.
Physical Demands:
While performing the duties of this job, the employee regularly is required to sit; use arms and hands to reach and grasp objects; talk and hear.
The employee frequently is required to walk.
The employee occasionally is required to stand; and stoop, kneel, crouch, or crawl.
The employee must occasionally lift and/or move up to 45 pounds.

Work Environment:
Normal work schedule:
8am - 5pm Monday through Friday.
Extended hours and irregular shifts required as needed.
Work is generally performed in an office with moderate noise level.
Local travel is required to conduct community outreach activities.
National and/or regional travel is required for training and conferences if a virtual alternative is not available.
Mileage for personal vehicle use will be reimbursed at GSA rates.
Use of personal mobile phone is required, as position entails substantial community outreach.
A monthly phone stipend is offered as part of hiring package.
